The Industrial Commission of Arizona revised two forms to include a fraud warning required under legislation signed into law by Gov. Doug Ducey earlier this year.
Ducey in April signed House Bill 2331, by Rep. Karen Fann, R-Prescott, to cut off indemnity benefits to workers convicted of filing a false claim. The bill requires an injured worker to be notified that a conviction for filing a false claim can result in a prison sentence, a $50,000 fine and forfeiture of all future indemnity benefits on a claim.
