Insurance Division Adopts Uniform Prescription Authorization Form

The Oregon Insurance Division adopted a uniform prescription drug prior authorization request form and rules that require payers to accept a request for prior authorization submitted on the form starting July 1. Former Gov. John Hickenlooper on July 1, 2013, signed into law Senate Bill 382, requiring all payers to use a uniform form to streamline and simplify the process for providers to request authorization before prescribing drugs.
