OSHA Inspection after Fatality Leads to $194K in Proposed Fines

State regulators in Wyoming cited a sugar plant for 39 health and safety violations at its facilities in Lovell and Torrington after a worker fatality. According to news reports, Lovell plant employee Anfesa Galaktionoff, 28, was killed in January when she fell through a floor opening at the Western Sugar Cooperative's beet wheel-processing pit. The Wyoming Occupational Safety and Health Administration inspected the plant twice immediately after the fatality and proposed $71,000 in fines.
