Forklift With Faulty Brakes Kept in Service for Seven Weeks, OSHA Alleges

A national farm supply company has been issued federal safety citations after allegedly requiring employees to keep using a forklift even after a brake failure caused it to crash into a customer’s vehicle at a facility in Xenia, Ohio. Rural King Supply Inc. faces proposed penalties of $258,672 from the U.S. Occupational Safety and Health Administration. OSHA issued citations for two willful violations on April 18.
