To reduce paperwork and perhaps spur more participation by doctors in the treatment of injured workers, the New York Workers' Compensation Board has adopted a more-universal form for billing and reports, the board announced.
The board is also offering training webinar sessions and a video to help familiarize providers with the new form.
Form CMS-1500 is the claim form used by Medicare and Medicaid and most health insurers, and it will soon replace eight other New York workers' comp forms, including:
C-4 and EC-4, doctor's initial report.
