DWC Posts Updated Subpoena Forms

The California Division of Workers’ Compensation posted to its website updated subpoena forms for use in district offices throughout the state.

The Workers’ Compensation Appeals Board has approved the new forms, the division announced on Tuesday.

Electronic copies of the forms that can be filled out on a computer are available from the DWC’s website, here. Hard copies will be available at the division’s district offices.
