The New York State Workers' Compensation Board created a new form as part of a change to the process used to update claimants' information.
The new Form OC-966, "Claimant Name and Address Form," should be submitted directly to the carrier, third-party administrator or self-insured employer, the board said.
Claimants' addresses are automatically updated in the electronic filing system when payers submit eClaims filings. Those filings overwrite any address update already processed by the board.
