A Valley City, Ohio-based metal-forming company is facing $89,500 in proposed fines after receiving Occupational Safety and Health Administration citations for the second time since 2013.OSHA inspected Superior Roll Forming Co. after a worker's hand was crushed in a 150-ton power press in November. The employee was removing a metal piece from the machine, which is used to assemble parts for automotive and other industries, when the accident happened.
