Board Requires More Info, New Forms for Impairment Eval

The New York Workers' Compensation Board has revised its reporting requirements and forms to include more details about injured body parts and impairment levels.

The changes are in keeping with impairment guidelines on schedule-loss-of-use evaluations that became effective this year and which are designed to clarify how physicians evaluate permanent impairment when maximum medical improvement is reached, the board announced Thursday.

Form C-4.3 now includes two attachments: If the patient has a permanent partial impairment, the medical provider should complete Attachment A for all body parts and conditions for which a schedule of loss-of-use award is appropriate, except for facial disfigurement or vision or hearing loss.

Attachment B should be used for all body parts and conditions for which a non-schedule award is appropriate. Both A and B should be completed when a patient has schedule and non-schedule conditions, the board said.

Also, independent medical exam forms have been revised to provide more information on the exam and the type of injury.

Two webinar training sessions on the new guidelines will be held Wednesday, and participants can register through the board's website.
