As part of its effort to align its paperwork with the more widely accepted Medicare requirements, the New York Workers' Compensation Board has revised two forms.
Form C-8.1, regarding treatment issues and disputed bills, and Form C-8.4, the notice of a carrier's refusal to pay a medical bill, have been updated to clarify legal and valuation objections, to remove obsolete sections and to move other passages to other forms.
