The U.S. Occupational Safety and Health Administration slapped a roofing firm in Taunton, Massachusetts with two willful violations, one serious violation and proposed penalties of $102,900 after a worker died when a lift boom he was operating tipped over.
The fall could have been prevented had Skyline Contracting and Roofing Corp. followed industry and federal safety standards, OSHA found.
