The City of Cincinnati violated 12 safety standards in June when an employee was electrocuted while installing a surveillance camera near electrical lines, the Ohio Bureau of Workers' Compensation said in a preliminary report this week.
Leroy Garrison, 48, was killed when he was working alone in a bucket truck and came into contact with the power lines, according to the BWC and a local news report.
