WCB Revamps Request for Hearing Form

The Oregon Workers’ Compensation Board has released a new Request for Hearing form and is asking applicants to start using it immediately. The new form has expanded the type of requests available and moved that section to the top of the form. Request types now include initial, for a new request for a hearing; supplemental, to add issues to a previous request for hearing; amended, to correct or add information to a request; and consolidated, to combine a request with a new case number with an active request.
