The California Division of Occupational Safety and Health issued a combined $529,640 in penalties to employers Smelly Mel's Plumbing and Sewer Rat Plumbing for violating safety regulations that resulted in a trench collapse that seriously injured a worker. A 30-year-old construction worker was injured on Aug. 1 while working on a sewer line project at a private residence in San Mateo.
