A young man lost four fingers and was severely burned on his first day of work because his employer failed to give him proper safety training, according to a news release from the Occupational Safety and Health Administration.
The failure to report the accident and a subsequent inspection by OSHA resulted in the third set of citations by the agency in five years Wednesday at the same plant for Quality Blow Molding of Elyria, Ohio, including a suggested $171,270 fine, according to the release.
