The U.S. Chemical Safety Board recommended strengthening process safety management regulations for petroleum refineries in a final report on its investigation into the August 2012 fire at a Chevron facility in Richmond.
The Richmond fire, caused by a ruptured pipe, created a vapor cloud that caught fire and engulfed 19 workers in flames. Vapors also caused 15,000 residents living near the refinery to seek medical treatment.
