OSHA Cites 'Serial' Offender for Fall That Killed Worker

A worker fell 22 feet to his death four months after the Occupational Safety and Health Administration cited his employer for failing to protect workers at the same Illinois job site, investigators said. Louisville, Kentucky-based Material Handling Systems/MHS Technical Services should pay $320,000 in fines for “serial disregard to fall protection,” OSHA said in a news release Monday.
