The absence of safety pins in two hydraulic leg stands and the lack of stationary jacks caused a mobile medical trailer to fall and crush to death a 58-year-old electrician on his first day of work for an Illinois manufacturer of custom trailers, OSHA said.
Investigators from the Occupational Safety and Health Administration cited Advanced Mobility by Kentucky Trailer, based in Monee, Illinois, with one serious safety violation following the July 5 fatality.
