Department Urges Compliance on Rehabilitation Forms

The Minnesota Department of Labor and Industry issued a notice Friday that qualified rehabilitation consultants are required to use service codes and categories in the development of "R-2 Rehabilitation Plan" forms and "R-3 Rehabilitation Plan Amendment" forms.The service codes and categories must also be listed on vocational rehabilitation invoices for billing insurers, as prescribed by the commissioner.The service codes and categories document is available on the department website.To reach the R-2 form go to http://www.doli.state.mn.us/pdf/re01.pdf.To reach the R-3 form go to http://www.dol...
