The Texas Department of Insurance updated its death benefits form to include a notice in Spanish on where to access an online version and how to contact the Division of Workers' Compensation for questions.
The department said carriers that use automated notification systems should update their processes to send the revised form.
The Beneficiary Claim for Death Benefits, or DWC Form-042, was updated May 22.
