The federal work safety agency has fined a marine cargo handling company just shy of $125,000 for safety violations that led to the death of a 48-year-old longshoreman.
In June, Jim Meadows was loading timber onto the Forest Trader, a 21,000-ton bulk carrier cargo ship docked in Weyerhauser County, Washington. He fell 10 feet onto the ship's metal deck and later died of his injuries.
The Occupational Safety and Health Administration cited SSA Marine, Meadows' employer, for failing to install netting or temporary platforms to protect workers from falls.
