Oregon’s Workers’ Compensation Division on Thursday posted revised forms that removed the field for providing the injured worker’s Social Security number.
Earlier in December, the division adopted new rules effective Jan. 1 eliminating the Social Security field on forms used to report work-related injuries and illnesses.
