Comp Division Posts New Forms Without SSN Fields

Oregon’s Workers’ Compensation Division on Thursday posted revised forms that removed the field for providing the injured worker’s Social Security number. Earlier in December, the division adopted new rules effective Jan. 1 eliminating the Social Security field on forms used to report work-related injuries and illnesses.
