OSHA Cites Bolt Maker after 2nd Death in Less than a Year

The federal Occupational Safety and Health Administration has cited Houston-based Madden Bolt Corp. after a worker died as the result of an injury at the company back in August, OSHA announced in a release. The death was the second one at Madden in less than a year, according to OSHA. The worker was killed when a cutting-table explosion that hurled the employee and a steel plate into the air and the steel plate landed on the worker, according to the release.
