Alleged Safety Violations Lead to $687,650 Fine

The Occupational Safety and Health Administration fined Champion Modular Inc. $687,650 for safety violations uncovered as part of an investigation into a November 2018 amputation.

OSHA cited Champion Modular for “willful and serious” violations that included failure to properly guard machines or provide fall protection at its facility in Strattanville. OSHA also said the company failed to train workers on hazard communication and hearing protection.

The company has 15 business days from receiving the citations to comply, request an informal conference with the OSHA area director or contest the findings before the Occupational Safety and Health Review Commission.
