Injured workers in Oregon who dispute a carrier's assertion that a claim is "nondisabling" have a new form to help them through the process.
The Oregon Workers' Compensation Division announced Monday that workers can now use Form 2943 to request review of an insurer's decision to refuse to reclassify a nondisabling claim to disabling.
The form, supporting documents, and any additional information to be considered should be submitted to the Appellate Review Unit, Workers' Compensation Division, 350 Winter St. NE, P.O. Box 14480, Salem, OR 97309-0405.
