Reminder Issued on Memorandum of Payment Form

The Vermont Workers' Compensation Division issued a reminder to stakeholders to be certain that they are using the correct memorandum of payment form. Vocational rehabilitation specialist Trudy Smith noted that the correct form has "Rev. 3/07" in the upper right hand corner, along with a telephone number and space for the total amount paid to date. All that information is needed when completing the memorandum of payment, Smith reported.
