Self-Insurance Applications Due July 31

Self-insurance applications, for new and renewing applicants, are due July 31, the Indiana Workers' Compensation Board announced. Forms and guidelines are now available online. Board chair Linda Hamilton said in a bulletin this week that it is “extremely important” that all questions on the application are answered and all attachments and additional information are included.
