WCS Posts Revised Forms

The Nevada Workers’ Compensation Section recently updated forms used to verify wages or release medical records. The WCS said it redesigned its Form D-8, “Employer’s Wage Verification Form,” so that it can be filled out electronically as a PDF.  The agency also made Form D-25, “Affirmation of Compliance with Mandatory Industrial Insurance Requirements,” a fillable PDF document and added new signature boxes and instructions.
