The U.S. Occupational Safety and Health Administration fined a tire distribution facility in Texas $288,299, saying proper workplace safety training and correct storage of pallets could have prevented a fatal forklift accident.
William Thornton, 39, died June 6 while working for Mitchell Industrial Tire Co. in Fort Worth, according to a report by KWTX.
