WCD Updates Claim Notice Form

The Oregon Workers’ Compensation Division revised its claim notice form to update language and formatting and to make the document usable for the employer-at-injury program. The division on Wednesday said it updated Form 3058, “Notice to Workers.” In addition to style changes, the updated form has a new reference to the Ombuds Office for Oregon Workers and a new email address for the Bureau of Labor and Industries.
