MIA Posts Medical Benefits Termination Form

The Maryland Insurance Administration has posted its new Insurer’s Termination of Medical Benefits form for electronic filing through the WFMS online forms filing system. MIA posted a link to the form on its website and said in a regulatory notice that the new form, introduced as C-10 in July, is also available on MIA's Forms and Instructions page. Code of Maryland Regulations 14.09.06.04 C requires insurers to provide written notification to the injured workers prior to the termination of medical benefits.
