The Maryland Insurance Administration has posted its new Insurer’s Termination of Medical Benefits form for electronic filing through the WFMS online forms filing system.
MIA posted a link to the form on its website and said in a regulatory notice that the new form, introduced as C-10 in July, is also available on MIA's Forms and Instructions page. Code of Maryland Regulations 14.09.06.04 C requires insurers to provide written notification to the injured workers prior to the termination of medical benefits.
