The U.S. Occupational Safety and Health Administration fined a wire drawing company in Texas $299,339 for violations that include failing to provide fall protection for its workers.
OSHA said WMC San Antonio allowed workers to ride on the top of an unsecured forklift attachment made to move wire mesh bundles around its plant.
The agency said one worker at the facility died while transporting bundles of wire to a flatbed trailer when the attachment slid off the forks and fell.
