The Oregon Workers’ Compensation Division released a new form for insurers to use when requesting reimbursement from the Employer-at-Injury program.
The updated Form 2360 raises the administrative fee to $180 from $120, effective Jan. 1. It also removes a 0.45 multiplier for wage subsidy periods with dates starting Dec. 31, 2019, or earlier.
