Pallet Maker Fails OSHA Inspection 6th Time, at Potential Cost of $152K

A Wisconsin wood pallet manufacturer faces $152,460 in penalties after the U.S. Occupational Safety and Health Administration cited it for safety and health violations a sixth time since 2011. OSHA inspectors returned to Wood Fibers Inc. in Niagara in October 2015 after the company failed to provide documentation that it had corrected hazards found earlier. OSHA said  workers were still exposed to the same dangers in the second inspection. The agency said it also found similar violations in 2011, 2012 and 2014.
