The Texas Division of Workers’ Compensation is accepting feedback on proposed changes to a form that allows prospective employers to ask the agency to check whether an applicant has previous comp claims.
The proposed changes would allow submitting DWC Form 156, “Prospective Employment Authorization and Certification,” electronically. The division also said requesters will not be required to notarize the form, although they will still need to certify they are eligible to receive the requested information.
