Form Changes Announced by Workers' Compensation Division

Florida's Division of Workers' Compensation has announced a notice of form change for self-insurers, insurers, claims administrators and medical providers.

The division has changed DFS-F5-DWC-9, deleting date references to the National Uniform Committee Official Data Specifications Manual and information an insurer or administrator previously had to enter.

In addition, the form completion instructions relating to ambulatory surgical centers is revised.
