Trench Collapse Death Leads to Heavy Fines for Excavation Firm

The U.S. Occupational Safety and Health Administration has hit an excavating outfit in Mason, Ohio with a $91,000 fine after a worker died in a trench collapse. Stauffer Site Services, LLC had failed to trench cave-in protection for its employees, which lead to a nearly 10-foot deep trench collapsing on the worker who was installing a sewer line, according to OSHA, which inspected the Hillsboro worksite in October 2015. The 26-year-old man died two days later of his injuries.
