A complaint about unsafe working conditions led to $76,000 in proposed penalties against a Watseka, Illinois metal fabrication business. The U.S. Occupational Safety and Health Administration said it issued citations for 15 serious safety and health violations to Lyon LLC after a Sept. 24 inspection.
